This emergency info sheet is perfect for the babysitter, or to keep on your refrigerator.
I remember a nurse telling me that if you are in an emergency, and fight or flight takes over, you can easily forget important information like your own address or phone number.
That’s why I created this emergency info sheet to write down essential information that I (or someone else) might need in case of an emergency.
And I’m sharing it as a free download for you today:
Here’s what to include when you’re filling out your sheet:
Important Information
If a neighbor or babysitter is in an emergency situation, they will need to know your address.
I’ve also included space for two cell phone numbers- in my case, my husband’s and mine.
Emergency Contacts
Here you can list the phone number for poison control, 2 additional emergency contacts, and any other important emergency information.
(Perhaps the animal poison control number, a preferred hospital, or the name of the kids’ school.)
Family members
In this section, you can write out the full names of all family members, their dates of birth, and any allergies, medications, or other important information that you may need to provide in case of an emergency.
Safety first
If you complete this form, make sure that it is only available to those you trust, like a babysitter.
If you’re not comfortable having it out on the fridge where everyone can see it, consider taping it to the inside of a cabinet or some other out-of-sight place where those who need it will still be able to access it if needed.
